What Is Adult Foster Care (AFC)?
AFC Services is a MassHealth program that are ordered by a Physician and delivered to a Member in a qualified home setting by a Qualified Caregiver. The Member will receive quality care at-home by a multidisciplinary team of Register Nurse services and oversight, AFC Provider Management, and qualified AFC Caregiver. Services include assistance with activities of daily living (ADLs), Instrumental Activities of Daily Living (IADLs), and other personal care as needed.
Who Is An AFC Member?
An eligible AFC Member is 16 years of age or older with medical or behavioral conditions that prevent him/her from fully taken care of self on a daily basis. Individuals who need assistance or support to perform the following task independently:

-A perfect alternative to institutional placement or residential group homes
-We assist individuals to live with dignity and independence
-We are a community-based program that allows members to be cared for in a Home-setting
-Have an Interdisciplinary team of RN’s, Care Managers and Social Worker Care Managers
-Caregivers receive a monthly stipend for providing daily care to individuals in need
